it's used on the inside of the TCH artwork.
I found it by cropping it out of the artwork and uploading that to Google reverse image search. Try that to find more, I don't have time for it.

If I remember correctly, those top two pages aren't on the vinyl artwork, nor the jewel case version of TCH. Is this the poster from the original digipak version? I'm really glad to see more TCH photo album artwork. I can't express how much I love that sleeve.

The girl 10 down and 4 over (the corner of the second block on the left side) is a still from The Social Seminar: Tom at the same part where the sample from the end of Chromakey Dreamcoat comes from:
around the 10:01 mark.
